Outlook Search showing multiple data files NO PST FILES

I am not sure this goes here but confused why Exchange 2010 has no forums so I am posting here in case I can get a response. 

I am running into a problem first uncovered while performing a simple Outlook search (Outlook 2010 by the way).  I did a search in current folder which I could not find anything so I clicked on "All Mail Items" and started to notice I was coming up with the same emails but under the Outlook Data File column there was one listed as for examplebsmith@domain.com and also Bob Smith which is the same person, same mailbox just one as the email and one the user's full name.  We do NOT use pst files so no issue there.  We do not use Offline Cache Mode either.  The same search from OWA does NOT show duplicates in the search.

I have wiped out the user MAPI profile and created new numerous times and no luck.  The one thing which is interesting after I open Outlook the first time after creating the new profile is there were 4 items in the Navigation pane such as the following:bsmith@domain.com, Bob Smith, Archive-bsmith@domain.com and Online Archive- Bob Smith.  There are 4 because we use the Online Archive feature.  After a period of time, I shutdown Outlook and re-open to find just two,bsmith@domain.com and Archive-bsmith@domain.com

This is causing problems while performing a saearch and maybe other issues not yet uncovered.  I do the same for the same user on other PCs as well as 2008R2 Remote Desktop Services all with the same result.
